Attention SWC churches, moderators, and treasurers:

The deadline to submit your information for the UCC Yearbook has been extended to April 11th for those who have not yet completed their reporting. There are also important updates/changes to the process for reporting.

The National UCC has created a Survey Monkey option that will collect all of the data normally collected from churches through Data Hub. If you have not already submitted your data through the Data Hub system, you must use the new Survey Monkey option, which is also a smoother and simpler process. Go to our Annual Church Reporting page for the link.

There is no longer a need for an access code. The only information you will need to begin is your Church Name and Church ID number. All Southwest Conference Church ID numbers are six digits long and begin with 80 (80XXXX). If you do not know your Church ID number, email Wende.

Beginning on page 16 of the updated Church Instructions you will find the worksheet which can be used to get your data all organized in one place before entry.
